29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press(es): On or about March 17, 2015, the employer did not provide and ensure the usage of point of operation guards on the press machines located in the EPTD Press Area of Building #1. Employees were exposed struck-by and caught-between hazards in the following instances;; (a) Oak Lam Press #30 (Model Oak Lam/Serial 42-LP-100-704) did not have point of operation guards installed and light curtain system was disabled. (b) Minster Piece Maker Press #51 (Model P2-60-36/Serial P2-60-23830) point of operation guard (finger gate guard) was by-passed. (c) Minster Press #60 (Model P2-60-32/Serial P2-60-19274) did not have point of operation guards installed. (d) Bruderer Press #53 (Model AG CH-9320/Type BSTA 30 Serial #8262) did not have point of operation guards installed. (e) Warco Press #59 (Model 100-2-72C/Serial 5514) did not have point of operation guards installed.
