29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. a. 4400 S. Packers - Department 7- On or about September 23, 2014, employees had access to a two outlet electrical box with open covers that had energized wires that were exposed and protruding outside the box on the post, near punch press # 17 in an aisle employees use to access equipment, thereby exposing employees to shock and electrocution hazard. b. 4400 S. Packers - Department 7- On or about September 23, 2014, employees had access to a dismounted Baldor motor junction box that had energized wires that were exposed and protruding outside the box, near punch press 35, thereby exposing employees to shock and electrocution hazard. c. 4400 S. Packers - Department 7- On or about September 23, 2014, employees had access to a control box with a missing start button on punch press 35 that had energized wires that were exposed, thereby exposing employees to shock and electrocution hazard. d. 4400 S. Packers - Maintenance Department - On or about September 23, 2014, employees had access to a fan electrical junction box with no cover that had energized wires that were exposed and protruding outside the box, thereby exposing employees to shock and electrocution hazard. e. 4400 S. Packers - Maintenance Department - On or about September 23, 2014, employees had access to an electrical junction box on the east wall with no covers that had energized wires that were exposed and protruding outside the box, thereby exposing employees to shock and electrocution hazard. f. 4000 S. Racine - Department 11 - On or about September 23, 2014, employees had access to an electrical junction box on a post in the main aisle, employees use main aisle to access the riveters. The electrical junction box had no cover with energized wires that were exposed and protruding outside the box, thereby exposing employees to shock and electrocution hazard.
