29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertain to their respective job assignments. a. A&D Wood Products, Inc. located in Elida, Ohio: On or about October 4, 2014, the employer did not ensure employees were properly trained on electrical safety related work practices prior to changing the fuses inside of a 240 volt electrical cabinet for the swing saw. Employees were not trained on the following topics: 1. The hazards associated with arc flash and arc blast; 2. How to select and inspect personal protective equipment to be used when working on or near potentially energized live parts; 3. How to effectively lockout and to verify the lockout of electrical equipment prior to performing work.
