29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: On or about August 5th, 2024, employees engaged in commercial bakery production activities located at 4657 G Street, Omaha, Nebraska 68117, were exposed to electrical and fire hazards in that production employees were getting electrically shocked, from an ineffective grounding of energized equipment. Instance of failure include but are not limited to: a) Energized electrical circuit between Twister #1 and E-stop (same side of the conveyor) on the Red Moline Line. b) Energized electrical circuit near Twister #2 and E-stop (same side of the conveyor) on the Red Moline Line.
