29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s). a) On or about May 11, 2016, the employer failed to guard the point of operation on the Cincinnati Press Brake, located in Building 155, exposing employees to struck-by, caught-in and amputation hazards. b) On or about May 11, 2016, the employer failed to guard the point of operation on the Summit lathe 20", located in Building 135, exposing employees to struck-by, caught-in and amputation hazards. c) On or about May 11, 2016, the employer failed to guard the point of operation on the Clausing 15" Lathe, located in Building 135, exposing employees to struck-by, caught-in and amputation hazards. d) On or about May 11, 2016, the employer failed to guard the point of operation on the Umaro Verical 53" Lathe, located in Building 135, exposing employees to struck-by, caught-in and amputation hazards. e) On or about May 31, 2016, the employer failed to guard the point of operation on the Meltog #1, located in Building 115, exposing employees to struck-by, caught-in and amputation hazards. f) On or about May 31, 2016, the employer failed to guard the point of operation on the Meltog #2, located in Building 115, exposing employees to struck-by, caught-in and amputation hazards. g) On or about June 1, 2016, the employer failed to guard the point of operation on the Pneumatic Press, located in Building 197, exposing employees to struck-by, caught-in and amputation hazards. h) On or about June 1, 2016, the employer failed to guard the point of operation on the Lidder Machine, located in Building 197, exposing employees to struck-by, caught-in and amputation hazards. i) On or about July 21, 2016, the employer failed to guard the point of operation on the Piranha machine, located in Building 155, exposing employees to struck-by, caught-in and amputation hazards.
