29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made: a. 418 Duncan Ave, Jersey City , NJ - Vehicle Maintenance Area Flexible cords used daily by employees to power hand held tools showed evidence of damage and had not been tested or removed from service until necessary repairs were made. Damaged cords were observed lying along the ground through wet areas. Violation observed on or about August 13, 2014
