29 CFR 1910.119(o)(4): The employer did not promptly determine and document an a ppropriate response to each of the findings of the compliance audit, and documen t that deficiencies have been corrected. The employer does not always ensure tha t suggestions, recommendations and/or corrective actions from compliance audits are resolved. A.The 2004 compliance audit documented that numerous P&IDs were ou t-of-date and inaccurate. This violation was observed on or about February 23, 2 010 when P&IDs, 18-E-54-3000, 19-E-54-1000, 19-E-54-3000, and 19-E-54-4000 were still out of date and inaccurate. B.The 2004 and a 2007 compliance audits each i dentified a need to resolve facility siting issues. This violation was observed on or about February 23, 2010 when facility siting issues were identified as remaining unresolved. This condition exposes em ployees to fire and explosion hazards. Pursuant to 29 C. F. R. 1903.19, within t en (10) calendar days of the date of this citation, the employer must submit doc umentation that it is in compliance with the standard, including updating the P& IDs and addressing facility siting concerns by reducing the exposure of employee s to hazards of explosions. One method of abatement among others is to have a se cond party verify completion of all recommendations/corrections.
