29 CFR 1910.119(m)(4)(iv): The report prepared at the conclusion of the investig ation of an incident which resulted in, or could have reseasonably resulted in a catastrophic release of highly hazardous chemical in the workplace, did not inc lude all the factors that contributed to the incident: This violation was observed on or about 1/19/2010, where all the perti nent facts concerning the formation of ice plugs in the gas lines and wells, tha t could have been contributing factors to the event on 1/13/2010, were not inclu ded in the undated incident report and causal analysis timeline. This exposed em ployees to fire, explosion and highly hazardous chemicals (HHC). Pursuant to 29 CFR 1903.19(d), within 10 calendar days of the date of the citation, the employe r must submit documentation showing that it is in compliance with the standard, including describing the steps it is taking to ensure that, when contract employ ees are involved, the contractor must be included on the investigation team.
