29 CFR 1926.1204(j): A system for the preparations, issuance, use, and cancellation of entry permits as required by this standard, including the safe termination of entry operations under both planned and emergency conditions was not developed and implemented. a) Williamstowne Curve (Access Road) Mobile, Al, on or about August 11, 2025: employees entered a sewer manhole containing a hazardous atmosphere without the implementation of procedures for the preparations, issuance, use, and cancellation of entry permits.
