29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings shall be effectively closed. (a) On or about July 19, 2023, at an establishment located at 121 Cedar Street, in Tigerton, Wisconsin; in the Debarker Cab, the green AB Allen Bradley box was not effectively closed. An employee would activate the trailer conveyor and hog buttons and was exposed to electrical shock and fire hazards. (b) On or about July 19, 2023, at an establishment located at 121 Cedar Street, in Tigerton, Wisconsin; on the control panel for the Mellott Tilt conveyor feeding the planer was not effectively closed. An employee would activate the conveyor and was exposed to electrical shock and fire hazards. (c) On or about July 20, 2023, at an establishment located at 121 Cedar Street, in Tigerton, Wisconsin; on the Green Line the start/stop button for the conveyor was not effectively closed. An employee would activate/deactivate the conveyor and was exposed to electrical shock and fire hazards.
