29 CFR 1910.255(b)(4):All press welding machine operations, where there was a possibility of the operator's fingers being under the point of operation, were not effectively guarded by use of a device such as an electronic eye safety circuit, two hand controls: Instance a) On or about July 14, 2023, the employer did not ensure that the #26 Taylor-Winfield manufactured spot welder (Serial #6847) was guarded to protect employees from point of operation hazards while being operated. Employees were exposed to burn and amputation injuries in the event the equipment was cycled with their fingers between the welding heads. Instance b) On or about October 12, 2023, the employer did not ensure that the #13 Federal manufactured spot welder (Serial #15260) was guarded to protect employees from point of operation hazards while being operated. Employees were exposed to burn and amputation injuries in the event the equipment was cycled with their fingers between the welding heads. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)
