29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: a) Production area - On or about June 27, 2018 and at times prior to, employees used an extension cord to provide power to equipment including but not limited to cell phones and fans. The ground pin was broken off the attachment plug, exposing employees to injuries from electrical shock.
