29 CFR 1910.178(q)(6): Powered industrial trucks were altered so that the relative positions of the various parts were different from what they were when originally received from the manufacturer, by the addition of extra parts not provided by the manufacturer, or by the elimination of any parts without the manufacturer's approval: On or about November 7, 2017, and at times prior to, employees were exposed to struck-by hazards when the employer made repairs to forklifts and safety related parts were not placed in the same position as located when the forklift was received from the manufacturer: a) Toyota forklift, model ZFGU55, serial number: 71854, horn button was installed on the dash of the forklift. b) Toyota forklift, model ZFGU55, serial number: 32187, horn button was installed behind the driver requiring him/her to reach behind the seat to use the horn.
