Safety signs, safety symbols, or accident prevention tags were not used where necessary to warn employees about electrical hazards which may endanger them, as required by 1910.145: Facility Wide: Safety signs, safety symbols or accident prevention tags were not installed on the outside covers of the premises wiring such electrical power panels, safety switches and control panels to warn employees about electrical hazards when they were required to perform repair, test, inspect, troubleshoot, and maintenance while the machines and equipment were energized.
