29 CFR 1910.219(i)(2): Bolt(s), nuts, or setscrews used in shaft coupling(s) extended beyond the flange of the coupling and were not covered with safety sleeve(s): a) Mill department: A rotating shaft for the motor on the Kobe warm-up mill had unguarded bolts that extended beyond the flange of the coupling, on or about 5/16/16. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
