29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings shall be effectively closed. a. On or about June 9, 2015, located in the Stair Bay, there was a 480 VAC electrical disconnect box that was used in part to supply electrical power to a jib crane where employees would manually turn electrical power on/off that had an unused opening on the side of the electrical disconnect box, thereby exposing employees to an electrical shock/arc flash hazard. b. On or about June 24, 2015, located in the paint area of the facility, there was a 480 volt electrical disconnect box that had a knockout removed. Employees test sprayed the block wall near the electrical disconnect and manually turned the power to the disconnect box on/off, thereby exposing the employees to an electrical shock hazard and/or arc flash hazard and when
