29 CFR 1910.265(c)(4)(vi): Walkway(s) and stairway(s), with standard handrails, were not provided where required in elevated and hazardous locations: (a) Sawmill - On or about March 6, 2015 guard rail system at floor opening to dust conveyor adjacent to edger was missing a mid-rail and the rail system did not extend the full length of the opening at its east end. Employees exposed to falling into pit. (b) Sawmill - On or about March 6, 2015 open walkway at dust chain cleanout pit near trim saw containing dust chain tail drum on west end of mill was not guarded. Employees exposed to falling into pit.
