29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: a. North side of subdivision entrance tower, On or about October 16, 2014, employees were exposed to falls greater than 10 feet in height to the ground level below, when working from the work platform of the scaffold and not being trained to recognize the hazard associated with the scaffold, while installing the cinder blocks to the wall. b. East side of subdivision entrance tower, On or about October 16, 2014, employees were exposed to falls greater than 10 feet in height to the ground level below, when working from the work platform of the scaffold and not being trained to recognize the hazard associated with the scaffold, while installing the cinder blocks to the wall. c.
