29 CFR 1910.146(d)(9): The employer did not develop and implement procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees, and/or for preventing unauthorized entry. a.) On or about August 23, 2023 and at times prior thereto, in building #1 dip tank pit area, employees entered and worked in the confined spaces. This exposed the employees to drowning, entrapment, and hazardous chemical atmospheres. b.) On or about September 28, 2023 and at times prior thereto, in building #2 dip tank pit area, employees entered and worked in the confined spaces. This exposed employees safety and health hazards.
