29 CFR 1910.219(e)(3)(i): Vertical or inclined belt(s) were not enclosed by guard(s) conforming to the requirements specified in paragraphs (m) and (o): a) Bagging Area (Inside Motor Housing Cabinets for "Two Pack" Bagging Machines) - Vertical and inclined belts for power transmission motors inside cabinets such as, but not limited to, DK-6091 were not guarded exposing employees to an amputation hazard. On or about March 13, 2012. Formosa Plastics Corporation, USA was previously cited for a violation of this same occupational safety and health standard 29 CFR 1910.219(e)(3)(i), which was contained in OSHA inspection number 311807374, citation number 1, item number 4a, and was affirmed as an informal settlement agreement on April 20, 2010, with respect to the same workplace located at 780 School House Road, Delaware City, DE 19706.
