29 CFR 1910.305(b)(1)(iii)(D): The raceway is sealed or plugged at the outer end using approved means so as to prevent access to the enclosure through the raceway: On or about August 5th, 2024, employees engaged in commercial bakery production activities located at 4657 G Street, Omaha, Nebraska 68117 were exposed to fire and electrical hazards in that multiple raceways were not sealed or plugged to prevent access through the raceway. Instances of failure include but are not limited to; a) Conduit located on Laminator #1 at the decline of the conveyor was not sealed or plugged in the Roll-in Room. b) Conduit located behind Roll-in Electrical Panel (#2) was not sealed or plugged on the top/bottom side of the conduit - Located in the Roll-in room. c) Mixer #4 Control Panel conduit was not sealed or plugged in the Roll-in room. d) Motor located behind mixer #3 conduit was not sealed or plugged in the Roll-in room. e) Conduit located under the conveyor on the blue line next to the blue over was not sealed or plugged. f) Conduit located below conveyor on the blue line going into the blue oven was not sealed or plugged - Located on the same side of Spiral #3 conveyor electrical panel. g) Conduit mounted on mixer #2 in the icing area was not sealed or plugged.
