29 CFR 1910.272(g)(2): When entering the grain handling facility bins, silos, or tanks from the top, employees did not wear a body harness with lifeline, or use a boatswain's chair that met the requirements of Subpart D, 29 CFR 1910.28: At the worksite located at 1315 North 2nd Street Clinton, MO: On or prior to January 29, 2024, employees were exposed to engulfment hazards. Two employees entered a grain bin five times without the use of a harness with a lifeline or a boatswain's chair. a) An employee, the Plant Manager, initially entered the grain to start flow. b) A second employee entered the bin to restart the flow of grain after it stopped up. c) The second employee made another entry after further flow issues and was engulfed in grain to chest height. d) The first employee entered the bin to attempt a rescue of the second employee with a truck driver that had arrived. e) The first employee entered again after the first rescue attempt was not successful and calling 911.
