29 CFR 1910.146(d)(3)(iii): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations, including isolating the permit space. a.) On or about August 23, 2023 and at times prior thereto, where employees were required to enter a confined space in building #1. This exposed employees to safety and health hazards when working in permit required confined spaces. b.) On or about October 13, 2023 and at times prior thereto, where employees were required to enter a confined space in building #2. This exposed employees to safety and health hazards when working in permit required confined spaces.
