29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made: On or about March 8, 2019, employees were exposed to electrical hazards from a yellow flexible cord used to power a Castex, Inc. (Model TR1500, S/N TR15000005321 SFS) floor scrubber that was not removed from service and the outer insulation was damaged exposing the inner wiring.
