29 CFR 1910.269(a)(2)(ii)(D): Qualified employees were not trained and competent in the proper use of the special precautionary techniques, personal protective equipment, insulating and shielding materials, and insulated tools for working on or near exposed energized parts of electrical equipment: Facility: On, or about, October 1, 2014, an employee who conducted electrical work in the facility, including live diagnostic work, was not adequately trained regarding what tasks required the donning of protective equipment to protect against shock and flash hazards. Protective equipment such as voltage rated gloves, face shield and balaclava were not worn when voltage testing was conducted to verify that electrical equipment was properly de-energized prior to working on the equipment
