29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven (7) feet or less from the floor or working platform were not guarded in accordance with the requirements specified at 29 CFR 1910.219 (m) and (o): a. 4400 S. Packers - Maintenance Department - On or about September 23, 2014, the Atherton Vertical Band Saw, Serial # 29940, pulley did not have a guard affixed, thereby exposing employees to the hazards associated with nip points and rotating parts. b. 4400 S. Packers - Department 7 - On or about September 23, 2014, the employer failed to guard the pulley on back end of the motor for the roll former on the Heim power press # 35, Serial # H3650, thereby exposing employees to the hazards associated with nip points and rotating parts.
