29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: (a) On or about 08/06/2014 - Facility, Toyota forklift (without a seatbelt), forklift was not taken out of service when the seat belt was missing, the horn was not operational, the seat was damaged, and the lights and backup alarm were not functioning.
