Text For Citation: 01 Item/Group: 012A Hazard:
29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. At Elberta Crate & Box Co., located at 606 Dothan Highway, Bainbridge, GA. 39818, on or about November 18, 2021, the employer exposed employees to shock hazards throughout the production area due to the following.: a. A flexible cord in the Boiler Room was "Daisy" chained together with another cord. This cord was used by the employees to power a portable fan. b. Cleat Mill #80, had a knockout box that was not mounted and the faceplate was not securely fastened to the outlet. Employees plug/unplug fans into the plug depending upon the temperature of the working environment. c. In Cleat Mill, the Knotching head and #41 Feeder Chain electrical boxes had openings that were not closed. This exposed employees who walk by to an inadvertent shock hazard.
