29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers: (a) 205 Young Street: On or about April 24, 2017 and times prior, employees were working near a 3 1/2 inch wide by 33 1/2 inch long floor hole while framing an internal wall of the residential building. (b) 205 Young Street: On or about April 24, 2017 and times prior, employees were working near and around holes 16in. from the floor and up to 13 7/8in. wide by 23 1/2in. long at the bottom of the steps leading to the basement.
