On or about 12/10/15 in the forklift and metal fabrication shops at 1301 New Savannah Road, Augusta, GA: Relocatable Power Taps RPT(s)(Surge Protectors) and flexible cords, connected to about 120 volts, were being used by the employer as permanent installed branch circuits, connected in series with other RPTs and connected to extension cords.
