29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: (a) Maintenance Shop: On or about June 25, 2015, and times prior, the extension cord used to power shop equipment and hand tools was missing a section of outer insulation exposing the insulated conductors. (b) West Virginia Grille: On or about June 25, 2015, and times prior, the electric wires that power the overhead lights in the restroom was missing the outer insulation exposing the insulated conductors. ABATEMENT CERTIFICATION IS REQUIRED FOR THIS ITEM
