29 CFR 1910.178(q)(7): Industrial trucks were not adequately examined before being placed in service. a). Throughout the Workplace- On or about and times prior to 20 February 2014, the employer did not ensure adequate inspections were conducted on forklifts prior to operation to include, but not limited to the trucks warning devices(e.g. reverse audio alarm, lights, horn), thus exposing employees to struck-by and crush-by hazards.
