29 CFR 1910.119(o)(2): The compliance audit was not conducted by at least one person knowledgeable in the process. a)On or about October 4, 2010 and at times prior thereto, the auditor conducting the compliance audit did not identify process safety management deficiencies and errors including, but not limited to: process safety information including consequences of deviation was not compiled prior to conducting a process hazard analysis, equipment did not comply with recognized and generally accepted good engineering practices selected by the employer, and existing equipment designed to codes no longer in general use were not documented to be safe.
