29 CFR 1910.119(e)(5): The employer did not establish a system to promptly address the team's findings and recommendations; assure that the recommendations were completed in a timely manner and that the resolution was documented: b- Hess Port Reading Refinery, Alky - The following PHA recommendations/remarks remained open and were not completed at the time of the inspection: 1994 node 24, p.8 - Verify that valves transferring material between caustic and water-wash sides of D-705R are chain-blocked or car sealed open. 1994 node 25, p. 212 - Consider adding fail position of LV-091 on P&ID. This recommendation appeared again on the 2004 PHA. 1994 node 26, p. 215 - Consider renaming caustic-side and wash-water side of D-705R. Both drums have same number for reference and this is seen as confusing. Also, consideration should be given to labelling the two sides of the drum differently in the field to indicate the caustic side and the wash water side. 1994 node 26, p. 222 - Check to see what indication is provided for pump running indication for caustic pumps P-707A/B and wash water pump P-708R. 1994 node 36 p. 319 - Consider updating P&ID 62-D8 to indicate inhibitor injection line 1"-A117-253 as out of service and blinded (closed). This recommendation appeared again on the 2004 PHA. 1994 node 38, p. 329 - Find out why hose connection on drains on PI-127 and PC-140 exist. 1994 node 40, p. 343 - Consider updating P&ID 62-D9 (vapor bypass valve on Debutanizer overhead line) to indicate that PV-140 fails closed. This recommendation appeared again on the 2004 PHA. 2004 node 46, p. 2 (Previous Recommendation) - Consider updating the P&IDs to indicate the pumps are provided with snuffing steam to the seals. 1994 node 40, p. 349 - Check vapor pressure of normal butane at tower temperature in the event of a control failure. 2004 node 32, p.8 - Consider installing spring-actuated, fail closed manual valves on all manual water drain lines in LPG service. 2004 node 16, p. 5 (Previous Recommendation) - Consider reviewing the use of the loop seal on the fresh acid tank overflow lines to ensure the seals are maintained. 2004 node 35 p.2 (Previous Recommendation) - Consider updating P&ID 62- D7 to remove the hose connections (e.g. on the deisobutanizer reflux pumps vent line. 2004 node 36, p.5 (Previous Recommendation) - Consider car-sealing closed valves, such as start-up fill lines, that are not used during normal operations and update the P&ID accordingly. 2004 node 38, p. 3 - See previous recommendation to consider removing hose connections. 2004 node 43, p. 8 (Previous Recommendation) - Consider car sealing closed the block valves on the crossover line (07-0-231B) between the isobutene tostorage and n-butane to storage to minimize the likelihood of cross contamination.
