29 CFR 1926.1204(b): The employer had not identified and evaluated the hazards of the permit required confined space before employee entry: On or about 05/01/2024 at the MTA-F line a 2.10-mile-long elevated steel structure from Kings Highway above McDonald Ave/Shell Road/West 6th Street and West 8th Street, abrasive blasters were bodily entering steel support column containments which were permit spaces. The employer did not identify and evaluate the hazards of the steel support column containments before abrasive blasters entered. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 AS PER 29 CFR 1903.19.
