29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Fabrication Area: Employees were exposed to an amputation hazard while operating the Pacific hydraulic press brake, model 400, serial number 3972, which was not equipped with point of operation guarding, on or about March 8, 2023. b) Fabrication Area: An employee was exposed to an amputation hazard while operating the green Pacific hydraulic press brake, model J90, which was not equipped with point of operation guarding, on or about March 8, 2023. c) Fabrication Area: An employee was exposed to an amputation hazard while operating the white Pacific hydraulic press brake, model J90, which was not equipped with point of operation guarding, on or about March 8, 2023. d) Fabrication Area: Employees were exposed to a crushing and amputation hazard while operating the Cincinnati shear, model 500 (white), which was not equipped with adequate point of operation guarding for the blade and hold down clamps, on or about March 8, 2023. e) Fabrication Area: Employees were exposed to a crushing and amputation hazard while operating the Cincinnati shear, model 1812G (green), which was not equipped with adequate point of operation guarding for the blade and hold down clamps, on or about March 8, 2023. f) Fabrication Area: Employees were exposed to a crushing and amputation hazard while operating the Accu-Shear machine, which was not equipped with adequate point of operation guarding for the blade and hold down clamps, on or about March 8, 2023.
