29 CFR 1910.305(a)(2)(ix): All lamps for general illumination were not protected from accidental contact or breakage by a suitable fixture or lampholder with a guard. Brass shell, paper-lined sockets, or other metal-cased sockets may not be used unless the shell is grounded. a) Stateroom 9 - On or about July 20, 2022, and times prior thereto, the employer had provided employees an aluminum shell light fixture for use without grounding, exposing employees to electrical hazards. b) Laundry Room - On or about July 20, 2022, and times prior thereto, incandescent light bulbs used for in a light fixture were not covered to prevent accidental contact, exposing employees to flying projectile hazards. c) Stateroom 11 - On or about September 29, 2022, and times prior thereto, two light fixtures used with incandescent light bulbs were not covered to prevent accidental contact, exposing employees to flying projectile hazards. d) Stateroom 2 - On or about October 25, 2022, and times prior thereto, an incandescent clamp light with aluminum reflective housing was used without cover prevent accidental contact. The fixture was also used without grounding, exposing employees to flying projectiles and electrical hazards. e) Stateroom 8 Storage - On or about October 25, 2022, and times prior thereto, incandescent light bulbs in a lighting fixture were not covered or guarded to prevent from accidental contact, exposing employees to flying projectile hazards.
