On or about 6/02/2022, at the Restaurant Depot facility located in Houston Texas, employees were exposed to chemical hazards of ammonia when the employer did not establish and implement written procedures to manage changes to replace a 2-inch carbon steel pipe from schedule 40 to schedule 80.
