29 CFR 1910.147(d)(2):The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a. Sanitation - On or about February 17, 2022, employees failed to follow lockout tagout procedures for the Turkey Carousel #2, exposing employees to a reenergization hazards. b. Sanitation - On or about February 17, 2022, employees failed to follow lockout tagout procedures for the Turkey Chiller Discharge, exposing employees to a reenergization hazards. c. Sanitation - On or about February 17, 2022, employees failed to follow lockout tagout procedures for Thin Slice Machine conveyor, exposing employees to a reenergization hazards. d. Sanitation - On or about February 17, 2022, employees failed to follow lockout tagout procedures of the giblet chiller, exposing employees to a reenergization hazards. e. Sanitation - On or about March 24, 2022, temp agency employees failed to follow lockout tagout procedures for the turkey processing equipment exposing employees to reenergization hazards. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
