29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure(s) at least annually to ensure that the procedure(s) and the requirements of this standard were being followed: Central Paper Stock, Inc. located at 6665 Jonas Place in Berkeley, MO 63134: The employer is failing to protect employees from caught-in and crushing hazards while performing maintenance, clearing a jam or cleaning of machines. This condition was most recently observed on about 11/17/2021, when the employer failed to conduct a periodic inspection of the informal energy control procedures used by employees to lockout machines such as, but not limited to, the following: a) The large "Harris Selco" baler (Model: HLO-8221AR-150; Serial No: 39770079) on the paper side of the plant; b) The small "Harris Selco" baler (Model:L125-2-10/8 ; Serial No:08138825 ) on the paper side of the plant; c) The "Retech" book grinder (Model: RG 52; Serial No: 7739101) on the paper side of the plant; and, d) The plastic "Badger" baler (Model: BADGER L-100S-2-10/7; Serial No: 8658) on the plastic side of the plant.
