29 CFR 1910.304(g)(5):The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: At the workplace located at 1020 West 29th Street, Kansas City, Missouri 64108, employees were exposed to possible shock, burns, fire or potential electrocution. An electrical shop light fixture with a three pronged plug was observed to have been plugged into a two socket extension cord. The grounding pin was ineffective and exposed.
