29 CFR 1915.74(a)(1): A gangway of not less than 20 inches walking surface of adequate strength, maintained in safe repair and safely secured was not used. If a gangway is not practical, a substantial straight ladder, extending at least 36 inches above the upper landing surface and adequately secured against shifting or slipping was not provided. When conditions are such that neither of gangway nor a straight ladder can be used, a Jacob's ladder meeting the requirements of (d)(1) and (2) were not used: a. On or about January 21, 2018, on the pier across from the electrical shop, employees were allowed to access a vessel by skipping the bumper tires attached to the pier, exposing employees to a drowning hazard.
