29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: On or about June 13, 2017, at the Art Street plant and main plant weld shop area, employer did not make sure there were no openings in the disconnect panel boxes. Employees exposed to an electrical shock hazard. The disconnect boxes were 120 volt boxes.
