29 CFR 1926.856(b): Floor openings shall have curbs or stop-logs to prevent equipment from running over the edge. a. On or about August 4, 2016, located at the southwestern portion of the 3rd floor of the former Columbus Municipal Light Plant, employees operated an Allis Chalmers, model number FT 20-24 PS, serial number 53348000 forklift where there was an 11 foot , 3 inch floor opening without curbs or stop-logs to prevent equipment from running over the edge into approximately 22 foot floor hole. b. On or about August 4, 2016, located on the 3rd floor of the where the eastern generator had been removed at the former Columbus Municipal Light Plant, employees operated an Allis Chalmers, model number FT 20-24 PS, serial number 53348000 forklift where there was a 7 foot, 4 inch by 12 foot, 4 inch floor opening without curbs or stop-logs to prevent equipment from running over the edge into approximately 12 foot floor hole.
