29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s): (a) On or about July 7, 2016, the employer failed to provide machine guarding to protect operator(s) and other employees from hazard(s) created by ingoing nip points and rotating parts associated with Iowa Precision Elbow Machines. Employees were exposed to caught in hazards in that the front panels/guards of Precision Elbow Machines were removed, exposing the interior moving hydraulic cylinders and rotating parts adjacent to the operator stations during operation of the elbow machines. (b) On or about July 7, 2016, the employer failed to provide machine guarding to protect operator(s) and other employees from hazard(s) created by ingoing nip points and rotating parts associated with manual collar machines. Employees were exposed to caught in hazards in that the point of operation guards were removed from the machines. Switches and foot controls are unprotected, allowing inadvertent activation of the collar machines. (c) On or about July 7, 2016, the employer failed to provide machine guarding to protect operator(s) and other employees from hazard(s) created by the point of operation of Riveter Machines. Employees were exposed to caught in hazards in that foot controls are unprotected, allowing inadvertent activation of riveter machines. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
