29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): A.) Falls Creek Powdered Metals, Inc., 82 Industrial Drive, Brockway, PA 15824, on or about November 23, 2015 and times prior thereto: Employees operate a Federal No.7 powdered metal press (Serial Number: 7-727, Machine Number: M1) for purposes including, but not limited to, sizing parts that were out of round. The equipment was equipped with guarding to the left side of the point of operation. The press was not equipped with guarding on the right and front side of the press, nor were tools and/or guarding devices used when performing hand-in-die feeding. B.) Falls Creek Powdered Metals, Inc., 82 Industrial Drive, Brockway, PA 15824, on or about November 23, 2015 and times prior thereto: Employees operate a powdered metal press (Machine Number: M2) for operations including, but not limited to, producing blades, cams and stamping serial numbers on parts. The press was not equipped with guarding or functional guarding devices when employees are performing hand-in-die feeding. C.) Falls Creek Powdered Metals, Inc., 82 Industrial Drive, Brockway, PA 15824, on or about November 23, 2015 and times prior thereto: Employees operate a Gasberre 30T powdered metal press (Serial Number: 95668, Machine Number: M19) for producing parts including, but not limited to, bushings for Toyota seats. The press was equipped with a guard that had a 5-inch by 18-inch opening which was too large. The opening allowed direct access to the point of operations which was positioned 11-inches from the guard. The guard was designed in that the employee could reach through the guard and access the point of operation. D.) Falls Creek Powdered Metals, Inc., 82 Industrial Drive, Brockway, PA 15824, on or about November 23, 2015 and times prior thereto: Employees operate a Fulesday MD40 powdered metal press (Machine Number: M13) for producing parts including, but not limited to, copy machine parts. The press was equipped with a clear plastic guard and an interlock device. The magnetic portion of the interlock device that should be connected to the door was disconnected to the door and affixed directly to the interlock on the machine. In addition, the opening in the guard was 6-inches wide by 7-inches in height, which was too large. The opening allowed direct access to the point of operations which was positioned 11.25-inches from the guard. The guard was designed in that the employee could reach through the guard and access the point of operation. E.) Falls Creek Powdered Metals, Inc., 82 Industrial Drive, Brockway, PA 15824, on or about November 23, 2015 and times prior thereto: Employees operate a powdered metal press (Machine Number: M6) for producing parts including, but not limited to, flanges. The press was equipped with an expandable metal guard that had a 3-1/2 wide by 2-inch high opening which was too large. The opening allowed direct access to the point of operations which was positioned 7-inches from the guard. In addition, there were no guards in place over top the guard to prevent access to the point of operation. The opening above the guard was 14-1/2 inches by 7-inches which was too large. The guard was designed in that the employee could reach through the guard and/or over the guard, and access the point of operation. F.) Falls Creek Powdered Metals, Inc., 82 Industrial Drive, Brockway, PA 15824, on or about November 23, 2015 and times prior thereto: Employees operate a Gasberre 60T powdered metal press (Serial Number: 87273, Machine Number: M5) for producing parts including, but not limited to, battery cable parts. The press was equipped with a removal expanded metal guard. While the guard was in place, the opening in the guard was 7-1/2 inches wide by 3-inches in height which was too large. The opening allowed for direct access to the point of operation which was 13-inches from the guard. In addition, the machine was not equipped to prevent access over the guard. The opening over the guard was 6-inches in depth which was too large. The opening allowed for direct access to the point of operation which was 10-inches away from the guard. The guard was designed in that the employee could easily remove the guard, reach through the guard, and/or reach over the guard, and access the point of operation. G.) Falls Creek Powdered Metals, Inc., 82 Industrial Drive, Brockway, PA 15824, on or about November 23, 2015 and times prior thereto: Employees operate a Fulesday 60T powdered metal press (Machine Number: M14) for producing parts including, but not limited to, bushings for boats. The press was equipped with a gated door that allowed for an opening 4-inches in height by 18-inches in width which was too large. The opening allowed for direct access to the point of operation which was 14-1/2 inches from the guard. The guard was designed in that the employee could reach through the guard and access the point of operation. H.) Falls Creek Powdered Metals, Inc., 82 Industrial Drive, Brockway, PA 15824, on or about November 23, 2015 and times prior thereto: Employees operate a Gasberre 60T powdered metal press (Serial Number: 99970, Machine Number: M16) for producing parts including, but not limited to, lawn mower parts and flanges. The press was equipped with a clear plastic and adjustable plastic guard that had an opening of 3-inches by 7-inches which was too large. The opening allowed direct access to the point of operations which was positioned 10-inches from the guard. In addition, there was a 6-inch by 22-inch opening overtop the guard that was too large. The opening allowed direct access to the point of operations which was positioned 16-inches from the guard. The guard was designed in that the employee could reach through the guard and/or overtop the guard, and access the point of operation. I.) Falls Creek Powdered Metals, Inc., 82 Industrial Drive, Brockway, PA 15824, on or about November 23, 2015 and times prior thereto: Employees operate a Gasberre 60T powdered metal press (Serial Number: 20001041, Machine Number: M17) for producing parts including, but not limited to, couplings and flanges. The press was equipped with a clear plastic and adjustable plastic guard that had an opening of 4-inches by 7-inches which was too large. The opening allowed direct access to the point of operations which was positioned 10-inches from the guard. In addition, there was a 6-inch by 22-inch opening overtop the guard that was too large. The opening allowed direct access to the point of operation which was positioned 16-inches from the guard. The guard was designed in that the employee could reach through the guard and/or overtop the guard, and access the point of operation.
