29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboard, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contained or deflected the falling objects: a. North side of subdivision entrance tower, On or about October 16, 2014, employees were exposed to falls greater than 10 feet in height to the ground level below, when working from the work platform of the scaffold, which was not provided with toeboard, while installing the cinder blocks to the wall. b. East side of subdivision entrance tower, On or about October 16, 2014, employees were exposed to falls greater than 10 feet in height to the ground level below, when working from the work platform of the scaffold, which was not provided with toeboard, while installing the cinder blocks to the wall. c. South side of subdivision entrance tower, On or about October 16, 2014, employees were exposed to falls greater than 10 feet in height to the ground level below, when working from the work platform of the scaffold, which was not provided with toeboard, while installing the cinder blocks to the wall. d.
