29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: a) Production area, milling area - On or about April 8, 2014, A flexible cable was connected to metal outlet box without proper strain relief, allowing tension to be transmitted to joints or terminal screws. A cable plugged into the outlet provided electrical power to a Kondia milling machine, exposing employees to a potential electric shock hazard.
