29 CFR 1910.146(k)(2)(iv): The employer did not ensure that affected employees practice making permit space rescues at least once every 12 months, by means of simulated rescue operations in which they remove dummies, manikins, or actual persons from the actual permit spaces or from representative permit spaces: a) At the establishment, on or about 4/15/15: Employer's written confined space entry program indicated authorized attendants would perform rescue operations. Affected employees had not practiced making simulated rescue operations at least once every 12 months. Abatement certification must be submitted for this item.
