A&D Wood Products, Inc. located in Elida, Ohio: On or about September 5, 2014, the employer did not ensure equipment such as, but not limited to, the 110 volt receptacle being used in the production area to power a salamander heater was approved for a Class II hazardous location. b. A&D Wood Products, Inc. located in Elida, Ohio: On or about September 5, 2014, the employer did not ensure equipment such as, but not limited to, a 240 volt receptacle being used in the production area to power the notcher was approved for a Class II hazardous location. c.
